MEDICAL SCRIBE


             Medical scribing is the process of transcribing a physician's medical notes and information during an office visit or procedure. This process is typically performed by a specially trained medical scribe who follows the physician and documents the visit in real time. The medical scribe is responsible for entering the data into the patient's electronic medical record (EMR) or health information management (HIM) system. The scribe typically records the patient's history, physical exam findings, diagnosis, treatment plan, and other relevant medical information, as dictated by the physician. Medical scribing is an important part of the medical documentation process, as it ensures accurate and timely completion of medical documentation.


Roles & Responsibility of medical scribe
           A medical scribe is a healthcare practitioner that works for a doctor, documenting patients and handling administrative duties. Medical scribes now spend most of their time working on an electronic health record (EHR) system in healthcare facilities that have switched from the outdated paper record system, however some still manually enter data in patient files.

Medical scribes, often known as medical transcriptionists, gather data about patients so their doctor can review it prior to the appointment. The demand for medical scribes has increased as a result of the overall digitalization of healthcare and the meaningful use stage 2 criteria, which demands that at least 50% of patients have electronic access to their health information.

By taking care of these auxiliary duties, the scribe frees the doctor to see more patients, spend more time with each patient, and think more deeply about complex cases. Without scribes, the doctor must record every detail, which may occasionally be overlooked or incorrectly documented. Inadequate patient care and misunderstandings in medical records may result from this.

The primary responsibility of medical scribing is to document the entire patient encounter as it happens. This includes entering patient demographics, transcribing the physician’s notes, entering orders into the electronic medical record (EMR), and entering the patient’s diagnosis into the EMR. Medical scribes also assist physicians with other tasks, such as ensuring that all necessary tests have been ordered and completing patient paperwork. Additionally, medical scribes may also provide administrative support, such as scheduling appointments.
